Search for a license
Section titled “Search for a license”- Enter License Number, or leave it blank and enter the exact full Name in the top form.
- Select Search. For a name search, clear any license number first; the number takes priority.
Partial names do not match. No result does not prove someone is unlicensed.
Match the person
Section titled “Match the person”Match the name, license number, NPN, and location. Results include License Type, Qualification, Issue Date, and Expiration Date.
The result does not make a current-status decision. Use the official state source and required carrier review to confirm current licensing and appointments.
